The 8-week course structure helps participants develop essential self-care skills and set achievable short-term goals. These goals are focused on better managing type 2 diabetes while maintaining an active lifestyle. Our program is designed to inspire confidence and motivation, assisting participants to navigate the challenges associated with living with a chronic health condition with greater ease.
To qualify for classes covered under bulk billing, participants should have a confirmed diagnosis of diabetes. However, we also welcome individuals at risk of diabetes or those who could potentially benefit from our program. While the content is mainly tailored for individuals with type 2 diabetes, those living with type 1 diabetes can also find value in our program.
We are proud of the noticeable improvements our participants have shown upon completion of the program. These include significant enhancements in strength, cardiovascular fitness, self-reported overall health, and well-being, as well as measurable reductions in blood pressure and blood glucose levels. After completing the program, we will inform your office and provide a detailed report of the participant's progress and engagement.

Overview of the 8-week Diabetes Education Program
Session 1: Understanding Diabetes and its Management
This session focuses on establishing the individual's current knowledge about diabetes, defining the condition, explaining its different types, predisposing factors, and common symptoms. We will discuss the aims of treatment and the crucial role self-care plays in managing the disease effectively.
Session 2: Blood Glucose Monitoring & Blood Glucose Ranges
This session will involve a comprehensive look at understanding and monitoring glucose ranges in the blood and urine. How to get referred?
Eligible patients can access the program through the Group Allied Health Services under Medicare. To do this, you'll need a referral from your GP or a relevant medical professional who is managing your diabetes care. They can provide you with a "Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes". Once you have this referral, you can join our program and start your journey towards better diabetes management.
